What can I expect at an appointment?
Physicals and well-child visits should be scheduled in advance to ensure you will be seen by your preferred provider and to make sure appropriate blood work is done.
Sick visits are scheduled the same day whenever possible. We advise you to call early for an appointment. Walk-in patients are seen on a first-come, first-served basis AFTER scheduled appointments and emergencies.
What happens if I am a new patient?
New patients need to arrive 20 minutes prior to the scheduled appointment time in order to register. Bring any forms that your provider must complete as well as the following:
1) Your insurance card
2) Photo ID
Do you have Imaging (x-rays, ultrasonography, CT-scan) facilities?
Yes, at our West Office, and Lawrenceburg locations, we offer:
CT (CAT) Scan
Bone Density Scan (DEXA)
Mammogram
X-ray
Do you examine children at your clinic?
Yes.
Our Family Care Division located at West Frankfort, East Frankfort, and Lawrenceburg see ages newborn to adult. At Lawrenceburg, we have a pediatrician, Dr. Jamie Cooper.
What happens if I need a prescription?
We require 24 hours’ notice to refill prescriptions. Please call at least two days prior to running out of your medication, in case there is a problem with your refill.
Insurance and Billing
We accept most major insurance carriers. Insurance patients must pay co-pays at time of service and have their insurance card available. We accept cash, check, or credit cards (Visa, MasterCard, American Express, and Discover).
As of October 1, 2016, all self-pay patients are required to make a $148.00 deposit prior to being seen. After your visit, you will be billed for the remainder of the charge.
We also ask that you kindly give at least 24 hours notice prior to your appointment if you need to make a change, in order to avoid being charged a $25 late cancel/no show.
